Here is the basic info for a request form for BOYD 2.0  please add
anything else we need.

Type of Computer (the more detail the better)
Purpose of Computer (no specify names)


I didn't think we need address, since they will have to pick it up.
Maybe we should include something about the organization having to pick
up the computer.

Maybe we should include a disclaimer that we will do the best we can,
but no promises.
